To become a patient of our clinic, you need to fill out the declarations selecting your doctor and nurse. You can complete this in person at our clinic with proof of identification. Declarations can also be downloaded from our website, completed, and then forwarded to our clinic.<\/p>

Without leaving the house, the patient can declare himself\/herself by submitting an E-DECLARATION<\/strong> to his\/her IKP Online Patient Account.<\/strong><\/a><\/p>

To do this, please complete the relevant application and sign it with a Trusted Profile or a qualified signature. The whole procedure is simple and should not take more than 10 minutes.<\/p>

From the moment of submitting the declaration with the simultaneous confirmation of insurance, the patient is covered by health care and may be provided with a free service under the general health insurance on that day.<\/span><\/span><\/p>

It is not necessary to submit the resignation to your current clinic.<\/span> The choice of a doctor may be done free of charge (twice a calendar year).<\/span><\/span><\/p>
