For this purpose, after logging in to the Patient Account <\/strong>website, in the MOJE KONTO [<\/strong>MY ACCOUNT] <\/strong>tab, you can submit an E-DECLARATION<\/strong> to the doctor and\/or nurse.
After entering MOJE KONTO [<\/strong>MY ACCOUNT]<\/strong>, scroll to the bottom of the page, and select TWOJA PODSTAWOWA OPIEKA ZDROWOTNA [YOUR PRIMARY HEALTH CARE] (POZ).<\/strong>Here are presented the possibilities of making a declaration to the doctors and nurses currently working in our clinic.<\/p>

If there is a doctor’s name there associated with our clinic – ul. Nowoursynowska 161, 02-787 Warsaw, it means that the declaration has already been made in another form, correctly submitted in our facility, and you can skip the next steps.<\/p>

If the field is empty or if there is a doctor outside\u00a0 of the list below, enter the new name of one of the doctors assigned to our facility \u2013 below is a list of doctors associated with NZOZ SGGW.\u00a0 Follow the instructions step-by-step<\/p>

CHANGE OF DOCTOR:<\/strong><\/p>

  1. In the Doctor box, click New Declaration \/ Enter Declaration.<\/li>
  2. Verify\/complete your data.<\/li>
  3. Select a facility:<\/li><\/ol>

      1. Choose a doctor (currently working POZ doctors in our facility) \u2013 you can choose one name:<\/li><\/ol>

                    1. Click “Podpisz deklaracj\u0119” [Sign Declaration] and follow the information that appears.<\/li><\/ol>

                      Once the e-declaration has been correctly signed, it will be submitted for acceptance in our clinic.
