  • The prescription can be ordered by calling the registration number – tel. 22 593-14-30 during opening hours of our clinic<\/li>
  • The service is provided only for patients who have submitted declarations of choice of doctor POZ in the Clinic NZOZ SGGW<\/li>
  • A prescription order can only be placed for solid medicines, as recorded in the card<\/li>
  • Please prepare a list of medicines \u2013 the registration worker has no insight into the patient’s medical history<\/li>
  • The prescription is issued for a period of not more than 4 months<\/li>
  • The deadline for the implementation of the need for medicines is seven working days. During the holiday period and during the pandemic period, the time limit may be extended<\/li>
  • Your doctor may refuse to list your medicine or write out a different amount of packaging than you ordered<\/li>
  • You do not need to have a paper version of the document when using e-Prescriptions. Simply enter your Social Security number and 4-digit passcode when visiting the pharmacy<\/li>
  • We recommend using an Online Patient Account – https:\/\/pacjent.gov.pl\/internetowe-konto-pacjenta<\/li>
  • By registering on the portal you will receive a prescription code via SMS<\/li>
  • Please note that if you have ordered more than 5 medicines, you will receive 2 codes in one SMS (the code is assigned to a maximum of 5 drugs)<\/li>
