Documents are submitted only by candidates qualified for admission. On the date of qualification, these candidates will be given the status of \u201cqualified\u201d, then the candidates qualified for admission to the second-cycle studies shall provide a complete set of required documents on the date indicated in the recruitment calendar. Please do not send the documents before the announcement of the qualification results.<\/p>

The following documents should be submitted by accepted Candidates (after qualification results):<\/strong><\/p>

1)\u00a0Personal questionnaire<\/strong>\u00a0with an\u00a0accepted photo<\/strong>\u00a0\u2013 a printout, available for download as a PDF file on the candidate\u2019s account at IRK in the \u201cEnrollment applications\u201d tab in the \u201cDocuments and further steps\u201d section. (proof of identity must be presented for inspection when submitting documents in person). Before printing the personal questionnaire, check whether it contains errors and whether it contains a photo. The candidate must sign the printed questionnaire in the lower right corner in the space for legible signature.<\/p>

2)\u00a0Certified by SGGW: Copy (the original document must be presented)\u00a0of higher education diploma entitling to undertake second cycle studies (Master\u2019s degree) in the country of its issue.\u00a0<\/strong>Document issued outside of Europian Union therytory must be legalized or legalization must be replaced by an\u00a0apostille<\/em>\u00a0certificate attached to it\u00a0\u00a0read more about legalization<\/strong><\/a><\/p>

3)\u00a0Certified by SGGW: Copy (the original document must be presented) of\u00a0oficial transcript of grades issued with the\u00a0higher education diploma\u00a0<\/strong>(a specification including the subjects taken, number of hours and grades).\u00a0Document issued outside of Europian Union therytory must be legalized or legalization must be replaced by an\u00a0apostille<\/em>\u00a0certificate attached to it\u00a0read more about legalization<\/strong><\/a><\/p>

4) Copy of\u00a0document confirming the knowledge of English\u00a0<\/strong>(the original document must be presented)\u00a0\u00a0READ MORE<\/a><\/p>

5) Copy of\u00a0valid passport\u00a0<\/strong>(page with photo) or\u00a0other document confirming the identity of the candidate\u00a0<\/strong>(the original document must be presented)<\/p>

6)\u00a0Certificate from an occupational medicine doctor (applies to candidates qualified for admission to the fields of study for which this type of certificate is required). After admission, the accepted canditate is\u00a0directed by the Faculty authorities for additional medical examinations to the occupational medicine doctor at the Polish medical facility, for this you have to print out \u201eSkierowanie na badania\u201d (referral for medical examination) which is available for download as a PDF file on the candidate\u2019s account at IRK in the \u201cEnrollment applications\u201d tab in the \u201cDocuments and further steps\u201d section. Accepted candidates are obligated to do this kind of medical examination as soon as possible after arrival to Poland and delivering a certificate to the relevant dean\u2019s office.<\/p>

7)\u00a01 up-to-date photographs\u00a0<\/strong>in accordance with requirements for the Polish ID\u00a0(format 35 mm (width)\/45 mm (height)<\/span><\/span>) and the same digital photograph (resolution 500 x 625 pixels)<\/span><\/span>\u00a0attached to the application in IRK System<\/p>

8) Declaration on health insurance during studying, declaration is available for download as a PDF file on the candidate\u2019s account at IRK in the \u201cEnrollment applications\u201d tab in the \u201cDocuments and further steps\u201d section.<\/p>

9) Copy of certificate from consulate confirming Polish descent or Polish Charter, if the candidate holds one (the original document must be presented).<\/p>

10)\u00a0Translation into Polish<\/strong>\u00a0of documents issued in languages other than Polish or English\u00a0\u00a0READ MORE<\/a><\/p>

NOTICE!<\/strong>
At the request of the Recruitment Committee Candidate is required to present the Recognition Statement issued by Polish National Agency for Academic Exchange including information confirming that the diploma gives the right to apply to second cycle studies (Master\u2019s degree).<\/p>
